RESUMO

1,196 consecutive cases were analyzed. The results show that familiarity is a major risk factor on goitre insurgence; minor risk factor is pregnancy. The most frequent thyroid disease in pregnancy is the lymphocytic chronic thyroiditis; if the women have familiarity for thyroid disease the probability to display a thyroid disease in pregnancy is most augmented. Thyroid scintigraphy shows its elevated sensitivity: only in someone thyroid adenomas a difference occurs between scintigraphic imaging and surgical-anatomical presentation. Antimicrosomal antibodies relief presents highest utility in autoimmune thyropathies.

RESUMO

Natural history of partial thyroidectomy for thyroid disease has been studied in a population of 1196 consecutive cases surgically treated from 1977 to 1982. Follow-up analysis on this population (from six to eleven years) was carried out for appearance of recurrence. Generally, the recurrence incidence was small (only 9.9% of total population) but high frequency was observed for Graves' disease (46%) and lymphocytic chronic thyroiditis (21%). The substitutive/suppressive therapy administration did not influence recurrence of disease. Our results suggest that suppressive therapy does not modify the natural course of non cancerous thyroid pathology residual to partial thyroidectomy.

RESUMO

Follicular, papillary, anaplastic and medullary cancers of the thyroid were investigated using immunohistochemical methods. The following antibodies were used: anti-S-100, antineuron-specific enolase (NSE), antikeratin, antithyroglobulin, anticalcitonin, anticarcinoembryonic antigen (CEA), antiepithelial membrane antigen (EMA); the following hormones were also tested in the medullary carcinoma: gastrin, ACTH and serotonin. Papillary and follicular carcinoma in particular reacted with anti-S-100 and anti-NSE; the anaplastic neoplasia reacted with anti-S-100 (25%), anti-NSE (12%), antikeratin (12%), antithyroglobulin (12%), anti-CEA (37%) and anti-EMA (37%). Medullary carcinoma reacted with anticalcitonin (100%), anti-CEA (96%), anti-NSE (79%), anti-EMA (4%) and anti-S-100 (17%). We were not able to correlate the virulence of the medullary carcinoma with the anticalcitonin and anti-CEA reactivity, while the hyperplastic C cells were immunoreactive both with calcitonin or with CEA.

RESUMO

Two cases of Boerhaave's syndrome are reported. They were treated by surgery within 12 hours from the onset of symptoms. In both cases the perforation was located on the anterior wall of the distal third of the thoracic esophagus. In view of the rapid onset of severe complications, particularly mediastinitis, the role of early diagnosis in relation to prognosis, is stressed.

RESUMO

A study is presented on the stage by stage survival curves in 153 cases of thyroid carcinoma observed over 15 years in Ancona Regional Hospital's General Surgery Division. The study reveals that prognosis is good in the first stage, but that neither surgery nor medical or radiological treatments is effective in preventing the deaths of incurable patients at later stages. It is therefore concluded that early surgery is more useful than any operations conducted at later stages.
